2026-27 PUSD Volunteer Clearance Application  - all the information you need to volunteer in school or for driving  carpool etc. Level 3 clearance is needed to drive carpool
​Parent Driver Transportation Info

Driver Transportation Form - needed to drive carpool, and must be accompanied by Auto Insurance Coverage Declaration Page with adequate insurance coverage (see Parent Driver Transportation Info link above)

Student Driver Transportation Form - for students to drive themselves to school events including sporting events/ games
